What is i2b2?
i2b2 (Informatics for Integrating Biology and the Bedside) is an NIH-funded National Center for Biomedical Computing based at Partners HealthCare System. The i2b2 Center is developing a scalable informatics framework that will enable clinical researchers to use existing clinical data for discovery research and, when combined with IRB-approved genomic data, facilitate the design of targeted therapies for individual patients with diseases having genetic origins. This open-source platform currently enjoys wide international adoption by the Clinical and Translational Science Award (CTSA) network, academic health centers, and industry. I2b2 is funded as a cooperative agreement with the National Institutes of Health.
The i2b2 Center is developing a scalable computational framework to address the bottleneck limiting the translation of genomic findings and hypotheses in model systems relevant to human health. New computational paradigms (Core 1) and methodologies (Cores 3) are being developed and tested in several diseases (airways disease, hypertension, type 2 diabetes mellitus, Huntington’s Disease, rheumatoid arthritis, major depressive disorder, inflammatory bowel disease, multiple sclerosis) (Core 2 Driving Biological Projects).

· Enables an informatics infrastructure framework to support the translation of basic medical research into medical practice.
· Reduces the time researchers spend in the discovery of research cohorts, study feasibility, recruitment of subjects and development of evidence-based protocols.
· Identifies potential cohorts for clinical trials.
· Aids in creating new research hypotheses.
· Performs retrospective data analysis.
· Gives researchers the ability to formulate complex queries using an understandable user interface and a set of well-defined patient characteristics.

Upon completion, the participants will be able to:
- Identify the experimental/epidemiological designs used in Next Generation Sequencing (NGS).
- Identify the concepts of read, pairing, alignment, indexing and Next Generation whole-genome sequencing.
- Identify genetic variation and mutation and structural variation from an assembled data set.
- Carry out basic de-novo assembly of NGS RNA-sequencing data.
- Perform basic tasks for NGS assembly and analysis using the GALAXY software system.
- Identify statistical procedures for analyzing the results of NGS assembly and data analysis (downstream analysis).

Abstract:
Clinical Logistics is an “embryonic field” that is defined as the work delivering to the right patient in the right environment, the right clinicians with the right knowledge, skills, and supplies at the right time (actually anticipating the right time) to do the right “thing” to achieve what the patient believes is the optimal critical care outcome. Key in the definition is the focus on the patient rather than where “logistics” is usually focused on materials or the function of units.
Thousands of researchers have focused on pieces of the above puzzle. Clinical decision support systems, electronic documentation, data visualization techniques, and equipment tracking systems are all parts of Clinical Logistics. This talk will argue that each piece of the Clinical Logistics puzzle must be understood both in the context of the entire current clinical system as well as being flexible and extensible into what should be a fascinating, and very different, near-term future.
About the speaker:
Dr. Jim Fackler might be considered an operational researcher within a hospital environment. One of his goals is to build a mathematical model of how patients flow through a hospital. For example, approximately half of patients who undergo surgery have been previously scheduled. These are patients who are expected and for whom resources can be appropriately prepared in advance. The other half of patients enter as emergency cases and therefore are less predictable. By modeling the external variability of the latter group, hospitals might become better able to predict the influx of emergency cases. Improvements in predictability would enable hospitals to optimize resource preparation and scheduling of elective procedures, thereby enhancing patient service.
Another research interest of Dr. Fackler is data acquisition and data mining in the operating room and intensive care unit. In the ICU, each patient has 300–350 associated data elements. Blood pressure, heart rate, potassium concentration, and hemoglobin level are just a few examples. Studies have shown that the human brain can comprehend 7 individual parcels of data at once. If the data can be sorted into patterns, an individual may be able to manage 7 individual patterns. Collaborating with some computer scientists and mathematicians, Dr. Fackler hopes to identify patterns among the patient data and establish recognizable trends that occur over time. The understanding of such patterns and trends could help clinical professionals to make more educated, evidence-based decisions on patient care.

The goal of this funding opportunity announcement (FOA) is to support collaborative translational research projects aligned with NIH efforts to enhance the translation of basic biological discoveries into clinical applications that improve health. This opportunity is specifically to promote partnerships between NIH intramural investigators (e.g., those conducting research within the labs and clinics of the NIH) and extramural investigators (e.g., those conducting research labs outside the NIH). It will provide support for extramural investigator to take advantage of the unique research opportunities available at the NIH Clinical Center by conducting research projects in collaboration with NIH intramural investigators.
While translating basic research into clinical practice is increasingly difficult, time consuming, and expensive, translational research is crucially important in converting basic scientific discoveries into new diagnostics and therapies for patients. As such, this FOA intends to broaden and strengthen translational research collaborations between basic and clinical researchers both within and outside NIH to accelerate and enhance translational science. Teams will have at least one intramural and one extramural investigator.
This program will provide access for external researchers to the NIH Clinical Center (CC), and will thus leverage the diverse CC resources, expertise, and infrastructure available to test promising laboratory- and animal-based discoveries with potential implications for disease diagnosis, treatment and prevention. The special environment of the CC supports studies that may not be readily supported elsewhere.
This FOA encourages high quality science demonstrating the potential to result in understanding an important disease process or lead to new therapeutic interventions, diagnostics, or prevention strategies within the research interests and priorities of the participating NIH Institutes/Centers.
